Why am I not able to make appointments or view my chart when I once was able to do so?
Your online account is not linked to your chart, hence you cannot make appointments online, see your chart online or request refills online. Please send us an online request using the General Comments Form
or call us at 281-325-4100 option 2 and we will immediately link your account to your chart. After your online account is linked to your chart, you will be able to utilize all functions of our website.
To better serve our patients and ensure all patients who go online can fully utilize all areas of the website we are requiring starting Dec. 21st, 2011 for all users to verify their identities. We apologize for any inconvenience this may cause. Please contact us at 281-325-4100 option 2 and we will immediately link your chart to your online account. To remedy the situation, we are also proactively linking charts to online accounts within 24-48 hours of patients registering or logging into our website. How do I change my email address online?
Your email address is tied to your chart as well as your online account. To change your email address, you must submit a request by completing our online Email Change Request Form
. Once your request has been submitted, your email address will be updated within 2 business days. It is important to alert us immediately when your email address has changed. Also do not register with your new email address on our website. This will create another account and you will not be able to see your email messages from your doctor.

For emergencies, please call 911 or go to the nearest emergency center. How do I make Appointments?
For urgent medical needs during office hours, please call 281.325.4100 to get a same day appointment or be directed to a nurse who will assist you. Should you need to speak to a physician after hours or on weekends, please call our office at 281.325.4100 to page the physician on call.
Should you need after hour services, visit our state of the art After Hours Center Monday through Thursday, 7p – 10p, Friday, 5p – 10p, Saturday, 8a – 5p. Treatments include minor illnesses, sports related injuries, gynecological exams for acute problems, laceration repairs, breathing treatments, animal bites, asthma, broken bones, minor burns, cold, cough, dehydration, eye irritations, respiratory ailments and many other services.
How do I request Prescription Refills?
We offer you the ability to make, reschedule and cancel your appointment at any time using our online
website services. You must be a member of the website to use this service. If you prefer calling, please contact our office at 281.325.4100.
When will I hear about my Test Results?
If you need an existing prescription refilled, contact your pharmacy or submit the request via our online
website service. You must be a member of the website to use this service. Once we have received the request, we will complete it within 2 business days.
Our commitment to you is to communicate all test results to you, whether normal or abnormal . You will hear from us by secure online message, by letter or by phone. Please allow 7 business days for your test results to be received and sent to you. You will be notified immediately if any of your test results are significantly abnormal. If you do not hear from our office within 7 days, please send a secure message to your doctor or call at 281.325.4100. How do I request a Referral?
What is an Annual Exam?
We work to assure you obtain referrals within 1 week of the request. You can contact us via online
web services or by calling the office at 281.325.4100. When you contact us, please include the specialty or specialist and the reason for the referral. We will notify you in writing when the referral has been approved by insurance. Please note that many insurances will only authorize referrals to providers on your plan. Should you wish to see a provider who is not on your plan, you will need to contact your plan's member services.
Why are Immunizations important?
An annual exam is a once-a-year visit to our office for a general health check. It generally lasts from 30 to 60 minutes. During this appointment you can expect your physician to review the following:
• General physical exam
• Update of family health history for any new serious illnesses
• Review of your health history
• Update of current medications and supplements
• Need for medication refills
• Evaluation of need for health screening tests based on age and personal and family history
• Update on immunizations
• Schedule follow-up visit to discuss new health concerns or address chronic health issues
If needed, will my doctor help me get admitted to a Hospital?
It is extremely important to us that you have regular immunizations as scheduled to help prevent getting sick or having unnecessary health issues. We work to remind you of when it is time to have immunizations done. Remember to bring your immunization records. Should the need arise, Physicians at Sugar Creek takes great care in coordinating admission to our preferred facility, Memorial Hermann Southwest Hospital. There, you will receive the same compassionate care as when you were in our office. All necessary information along with contact to the hospital will be arranged per your health plan guidelines. If you are admitted through the Emergency Center , the physician on call will see you and make the necessary arrangements for your admission. Upon discharge from the hospital, you can easily have an appointment made and follow-up with your physician at our office. What types of Insurances are accepted at the clinic?

A resident is an individual who has graduated from an accredited medical school and is obtaining three years of post-graduate training in the specialty of Family Medicine. Resident physicians work closely with experienced faculty physicians so that patients receive the benefit of up-to-date medical knowledge and experience in treating their conditions.
